Choose how you wish to sign up
Option 1	Send your application form to John electronically by filling your details in online and send your money using paypal. Please add an extra 5% to cover the paypal charges and 50p to cover the stamp and envelope. This makes the final amount payable £14.70
Option 2	Download the application form, print it, fill it out and post it to Angie with either a cheque or postal order. The form is available for download as a PDF or a plain text file. If you use the text version please make sure it prints on one sheet of paper. Please include a stamp addressed envelope with your application. To be able to read PDF files you need a PDF reader.
